GENERAL INFORMATION
First time office visits:
Please arrive 30 minutes early for
your first scheduled appointment to allow time to fill out forms
regarding insurance and demographics and to have your lab work
drawn, if needed. Please be sure to bring a copy of your
insurance card(s) with you. You may print and fill out to
bring with you the Assignment of Benefits form from the “Forms”
option on this website.

Shot clinic and infusion area:
If you are scheduled for a
prolonged treatment, we suggest you bring a video (VCR or DVD),
reading material, personal stereo or some other items to help
you pass the time. You may want to bring something to eat if you
feel you will get hungry. Complementary snacks and beverages are
provided for your convenience. We also have a patient library
with reading and video materials that may be of interest to you.
Children under the age of 14 are not allowed in the infusion

Return appointments:
If you need a return appointment, please
make sure you have been given the time, date, and location
(Seton NW, Central, or South office, etc.) of your next appointment(s) before you leave the office. Please note that
appointments are made not only for follow-up physician visits,
but also for laboratory studies, injections, chemotherapy
treatments, social service consultation, nutritional
consultation, and research studies. Please arrive 15 minutes
early for return appointments to allow time for any insurance
updates and lab work. (back

Missed appointments/rescheduling:
If you are unable to keep
your appointment, please call us ahead of time at (512) 421-4100
to make other arrangements. If you are calling long distance,
you may call (800) 856-3963 toll free. Please notify us promptly
when you cannot keep an appointment. This will allow that time
to be used by someone else in need.

After hours and emergencies:
Your physician, or one of
his/her associates, is available 24 hours a day, seven days a
week. If you need to speak with a physician after hours, on
weekends, or on holidays, please call the Travis County Medical
Exchange at (512) 483-9036. The Medical Exchange will then page
the physician on call. It is helpful to have your pharmacy phone
number available in case the physician needs to call in a
prescription for you. (back

Prescription refills:
Many of your prescriptions may be
refilled over the telephone. Simply call your pharmacist with
the name of the medicine and the prescription number (which can
be found on the medicine bottle). Your pharmacist will then call
our office for refill approval. Please try to phone in your
requests for refills early in the day during office hours.
Certain controlled medications (such as narcotics) are
required by law to be written on special prescription pads and
cannot be filled over the telephone. You, or a responsible
party, will need to come by the office to pick up the
prescription and take it to your pharmacy. Thus, it is very
important that you check your medication supplies prior to
weekends and holidays to be certain you have adequate
medications to last this duration of time. Please be sure to
request refills of your medicines while you still have a few
days of medication on hand.

Other:
Our facilities are “clean air” facilities. Smoking is
not permitted either inside the buildings or within 18 feet of
the entrances. We appreciate your cooperation with this policy.(back
